Counting the Uncounted: Population-Level Surveillance of Documented Pregnancy and Fetal Harm in Police Crash Narratives with a System One Model (Jev) (arXiv 2610.00213)

A population-scale surveillance study in which Jev reads every Texas police crash narrative from 2017 through 2025, 5,018,079 in total, under an eight-question schema. It estimates 5,467 crashes with a documented pregnancy, including 58 documenting fetal harm.

Highlights

  • Reads every Texas police crash narrative from 2017 through 2025, 5,018,079 in total.
  • A regular-expression screen flags 6,840 candidates; Jev confirms 5,333 as documenting a pregnant person.
  • The same model reads 499,306 of the remainder under the presence question to estimate missed cases.
  • Blind human adjudication gives weighted sensitivity 0.999 and specificity 1.000.
  • Person-level records give 1.13 to 1.45 per 1,000 female drivers aged 15 to 49.

Watch out

arXiv preprint posted September 21, 2026 (stat.AP) by Amir Rafe and Subasish Das. The 5,467-crash estimate depends on the Rogan-Gladen estimator and a probability-stratified review sample, and narratives document only 2.56% to 3.09% of expected pregnant drivers, so the estimate is a documented-case figure rather than full prevalence.

A system that joins coded and narrative views of 5,601,890 Texas crashes from 2017 to 2025 into population estimates: Kumo Tabular reads coded records, Jev reads narrative samples, and human judgments recalibrate the probabilities. A system that codes Texas crash narratives as gated, typed Jev decisions returning probabilities and no text: a screen covered 499,500 narratives and 195,857 were coded with a 27-question schema. The typed model attains F1 of 0.908 against human labels, and recalibration cuts calibration error 3.3-fold.

The request shape barely changes across surfaces; the surrounding contract does. When to call the API directly and when a gateway, a Worker binding, or a router entry earns its extra hop.
